Campus Visitor Access Form

All members of the university community are required to access the online portal and pre-register any visitors prior to their arrival on campus.

CARE Referral Form

Submit a referral here to connect any WFU student, faculty, or staff member—including yourself—with supportive campus outreach.

Clery Travel Report Form

This form must be completed by any faculty or staff member responsible for supervising students during off-campus trips and sanctioned travel.

Community Input Form

Utilize this form to submit your comments, feedback, or suggestions directly to the University Police Department management team.

Complaint Form

Complaints against a police department employee. Submissions are taken seriously and forwarded to management for review.

CSA Reporting Form

Faculty and staff designated as (CSAs) must use this form to fulfill their federally mandated obligation to report qualifying crimes to the University.

Please utilize this service request form to secure dedicated police personnel or event security officers for an upcoming university or private special event.

Request a Police Report

The department adheres to public records guidelines; therefore, only public information segment of any requested report is eligible for release.

Ride-A-Long Release Form

Participation in the Ride-Along Program is contingent upon the successful execution of this Release of Claims and Assumption of Risk Agreement.

Trespass Appeal Form

To contest a trespass warning, individuals must submit a formal, written appeal within ten business days from the date the warning was issued or received.
